On March 21, 2014 I completed a survey for this company that presented themselves as OnDemand Research. They sent me a form to complete and mail to their address. In return, I would get a $25 gift card.
It is now June 23, 2014 and I have yet to receive any gift card. I believe it is a scam, probably to get your information.
I don't even know that such a company exists because if you Google them, you will only see references for this website, Ripoff Report. Apparently, they have been doing this since 2013, and they need to be found and stopped.
You filled out a form and mailed it in, but apparently that's just hurdle #2 (hurdle #1 is doing the surveys). What you filled out was their "Claim Form". That just gets you access to the "Prize Validation Form", hurdle #3. The Prize Validation form asks for:
Provide all information requested below, truly & accurately
Sign the Prize Validation Form (required). By signing you agree to receive additional offers by email,
phone and/or U.S. mail.
Attach a copy of a validly issued photo ID (Driver’s License, State ID Card, Passport, etc.)
Mail this original Prize Validation Form with attached copy of valid ID within 60 days, from the date
you took the survey, to:
Surveys Samples & Savings, 1303 US HWY 127 South, Suite 402-113 Frankfort, KY 40601, United
States.
You will receive your $25 Gift Card within 8-12 weeks following our verification process that confirms
you have met all requirements of the Gift Rules.
I don't know if they're legitimate or not, but what is a fact is that they make you go through a LOT, I mean A LOT to get that $25. And I'm a bit uncomfortable sending a copy of my drivers license or passport.

This company tries to weasle out of the promised reward by trying to impose new terms and conditions after the fact, I completed the survey and sent in the "Claim form" which clearly states as shown below, but yet they are trying to unjustly get more for the reward than was originally agreed to BY USING THE THREAT OF NOT SENDING THE GIFT YOU EARNED IF YOU REFUSE THE ADDITIONAL TERMS AND CONDITIONS, things such as;
- have to provide a photocopy of personal ID
- Have to wait another additional 8-10 weeks
- Agree to receive additional offers by email, phone and/or U.S. mail.
and new conditions:
"Mail this original Prize Validation Form with attached copy of valid ID within 60 days, from the date you took the survey",
(this above mentioned clause and condition was not a part of the original terms and is not possible to satisfy since they take longer than 60 days from the date of the survey completion to provide this form.)
The Claim Form States as follows:
I am submitting this Claim Form with the understanding that IT (meaning it, its contents and nothing else) WILL BE RELIED UPON TO VERIFY AND CONFIRM MY ENTITLEMENT to receive the sponsored Gift by Surveys Samples & Savings . (meaning this form and the info entered which confirms I fulfilled my end of requirements in full is all that is required, not this form AND ANOTHER NOT PREVIOUSLY UNDISCLOSED NEW ONE WITH ADDITIONAL NOT PREVIOUSLY UNDISCLOSED CONDITIONS AND TERMS)

Surveys Samples & Savings reserves the right to substitute my gift with a gift of equal or greater value depending on the availability of the original gift. I understand that I will be disqualified from receiving any gift which may be awarded to me if any statement made by me on this Claim Form is false and/or incorrect
