Received numerous letters suddenly stating the following:

"We're sorry to inform you that your insurance coverage lapsed because we didn't receive your premium payment by the date indicated above. But you can still save your important protection if you act immediately. All you have to do is mail in the payment notice below plus your check or money order for the quarterly, semi-annual or annual premium shown above to National Union Fire Insurance Company of Pittsburgh, PA in the enclosed envelope.

Mail your payment to us immediately....

It goes on to list four different payments that could be made - quarterly 7470 - semi annually - 149-40 or annually - 298.80.

Goes on to give the time and day that they are available to talk to and the number where they can be reached.

Since I know who my insurance company is and has been for 9 years now, I knew this was a fraud.

Through regions Financial corporation I purchase a personal injury policy in the amount of $9.95 monthly.

The policy number is 51442452. I was unaware of having this policy due to an incorrect transaction on my checkingaccount I learn that I had this benefits for reimburse of personal injury inpatient hospital stays reimbursed in the amount of 1500.00 per day.

unfortunately, these company does not want to provide reimbursement. I learned that i had this protection and submitted an apllication for reimbursement and was denied because 90 days of this event the claim should have been filed.

I never received a copy of this policy therefore I had no knowledge until the bank error. Immediately I filed as soon as I was informed by Regions customer service. On 06/27/2011 to 06/29/2011 treatment dates indicates Observation room charges incurred so they don't pay for observation room. I was treated as if I was inpatient with all the treatments as being inpatient and physical therapy was provided because I fell in my room the St. Vincents hospital took x-rays and discovered knee effusion. But since I was in there dr discovered other medical illness and I was treated for those deceases as well.

06/13/2011 has I total right knee replacement due to an on-the-job injury in 2007 the OWCP took so long to evaluate and decided to perform surgery due to and recent injury performed on 12/10/2010 which is the policies effective date of benefits. that was three days on this inpatient stay...

I have filed three appeals regarding this denial... they said they spoke to me saying I legally agreed to purchase this coverage. Now they are saying my policy lapsed. When on each time submitted through my bank was returned intentionally. The money was there. May 30.2013 is the first time receiving policy benfits information,. In this recession

why would I not submit inpatient stay if I knew of this existence. two claim numbers 96130523 and 96115156. If my disability insurance povided reimbursement as if these were injury so should they be made to keep their word. Since 2010 they should be made to give me allback they intercepted from my checking account. and underhandedly lapsed my policy

My mother has been a Macys customer and card holder for many years. She is 84 years old but is still very independent and is able to manage her own affairs. She noticed a 16 dollar charge appearing in her Macy's bill around 4 months ago, she called Macys to inquire what this charge is for and she was told that Macys had no knowledge of this charge. The Macys representative then gave her a phone number to call.

Only when she called the number she was given did she discover the charge was from the National Union Fire Insurance Company for a policy she had never requested or signed up for.

The individual she spoke told her it 'this thing is here from 2007'...my mother then asked him how was it that if it was a charge from 2007 she was only now getting billed. She then asked to speak to a supervisor and was told that someone would call her back.

Then a woman called her the next day and my mother explicitly told this individual that she had never authorized this charge and wanted it removed from her Macys bill. The Supervisor then assured my mother, after apologizing many times, that this would not happen again and told her that she would be receiving a letter from her within 10 days.

What my mother received was a letter from the National Union Fire Insurance Company of Pittsburgh PA. Signed by a Mr. Mark Fraser a Sr Vice President from Direct Marketing stating that enclosed was a change of Beneficiary form. Again this was not what my mother requested.

Although I am filing this fraud/complaint against the National Union Fire Insurance Company of Pittsburgh PA for charging a person for a bogus policy, it seems to me that maybe the bigger fault here has to lie in Macys lap. How can they insert a line item on a customer’s bill for something they know nothing about?

I am not sure if this report I am filling will do any good. I believe that my mother has been swindled out of those 64 dollars, which for some people is a measly sum but for an elderly person on a fixed income it’s a great deal. Both companies are complicit. But maybe Macys is a little bit more so, as they allowed fraudulent charges to appear on a customer's bill.

I am advising my mother to send back to the change form sent to her by the National Union Fire Insurance Company of Pittsburgh PA and simply write across...Cancel Policy. Then go back to Macys and file an official complaint there. I think she'll have to cancel her charge card at Macys as well. If there is anything else that should be done I would appreciate some feedback.
