I just want my Medical Records. Can Jeanette Robbins please refer all former patients to a specific trustee for collecting our personal medical records, pursuant to federal and state law? Please?

I really don't care about her personal mis-use of business funds. I do care about how my medical records are floating around in some storage unit somewhere.

Has anyone had any luck in retrieving their medical records from

In 2009 & 2010 Jeanette Robins did not pay payroll taxes withheld from employee paychecks. She tried setting up a payment plan with the IRS to pay down what was owed but was not able to keep the arrangements.

Under the advice of her attorney, Jeanette dissolved the Sleep Institute of Utah in 2012 & had taxes applied to her personally. Part of agreement was not to run a business again. The agreement was finalized in 2013 with IRS agent, Suzette Bybee 801-626-0736 & cell 801-918-3267.

12 Alice machines were sold to MedBridge Healthcare in April 2012 as part of a management agreement. When Management agreement terminated in Jan 2013, MedBridge rented the equipment back to Sleep Institute. The Alice equipment sold is owned by MedBridge Healthcare.

No taxes paid or reported for payroll from August 2014 to November 2014 when all employees laid off.

All employees called around 8pm on Sunday, November 2, 2014 by her husband Matt Robins. He informed them that they were laid off & there was no money to pay them their paycheck for their normal payroll day of November 3, 2014.

Between August & November of 2014, no paystubs were provided to employees. They were given hand written checks that often bounced or were late. When employees pushed for a paystub, the records they received did not match amount written on their check nor the time clock records.

Jeanette used ASBA non profit bank accounts & Sleep Institute bank accounts for personal use. She often used the debit card to pay her home utilities, children’s school necessities, family groceries and vacations.

In October 2014 Jeanette began writing checks from multiple bank accounts knowing the money would bounce. She wrote the checks & transferred the funds to a “safe” account the same day they were deposited and then allowed the checks to bounce. Attached are copies of two checks she wrote to the ASBA non profit account and a check written from the ASBA account to her husband. The total of these 3 checks is over $10,000. The bank account was originally negative over $7000 but after deposits made to the account, it was negative $4500 when the bank agreed to transfer the balance to a loan & set up a payment plan to pay it back to the bank. At this point, the account had been left negative for 45 days and was on the verge of charging off.

No W2’s have been sent out for employees from 2014

In October 2014 patients paid co pays up front for their appointments but the business closed down & they were not notified & showed up to their appointments. The patients tried contacting the office to get a refund but have not been able to reach anyone.

Patient records for their sleep studies have been lost and no way for referring physicians or patients to retrieve their results or records.

Jeanette hired Lisa Cypers Kamen to help with the Sleep & Wellness magazine. After working with her, she extended her work with the Sleep & Wellness Conference in 2014. Once it was over, Jeanette was upset with Lisa & the outcome of her work. Rather than discuss it with her, she instructed the Accounts Payable department not to pay her & to make sure everyone she knew & worked with was paid so that if she asked she would know she was the only one not paid.

Jeanette had leases with 2 locations that she struggled to pay the rent with. Rather than talking to the landlords, she hired moving companies to come in on a weekend or in the evening to pack everything up & leave the landlord without notice.

There is a pattern of her walking away from debts, dissolving the company to protect herself and leave the person/company owed money no way to recoup the money.
